上皮样肉瘤 (Epithelioid sarcoma)
上皮样肉瘤是一种罕见的恶性软组织肿瘤,组织来源尚不完全明确,可能起源于原始间叶细胞。该病多见于青少年和年轻成人,好发于四肢远端,尤其是手指、手掌、前臂等部位。临床上常表现为皮肤或皮下的无痛性结节,生长缓慢但易局部复发和淋巴结转移。病理特征为肿瘤细胞呈上皮样形态,常表达细胞角蛋白和波形蛋白,部分可见SMARCB1(INI1)基因缺失。诊断依赖组织病理学及免疫组化检测,治疗以手术扩大切除为主,辅助放疗和化疗效果有限。近年来,基于INI1缺失的表观遗传治疗和靶向EZH2的临床试验为晚期患者带来新希望。
