塞扎里综合征 (Sezary syndrome)
塞扎里综合征是一种侵袭性皮肤T细胞淋巴瘤，以红皮病、淋巴结肿大及外周血出现克隆性恶性T细胞（塞扎里细胞）为特征。发病机制涉及Th2型细胞因子失衡与免疫逃逸，导致表皮浸润性亲表皮性T细胞增殖。典型临床表现包括广泛性红皮病、剧烈瘙痒、脱屑、掌跖角化、甲营养不良及淋巴结肿大，晚期可累及内脏。诊断依据外周血流式细胞术检测CD4+/CD26-或CD4+/CD7-细胞，结合皮肤活检病理及TCR基因重排分析。治疗采用针对皮肤的光疗、免疫调节剂（如干扰素α）、靶向CCR4单抗mogamulizumab，以及符合条件的造血干细胞移植。该病在精准医学研究中关注分子分型与新型靶向药物开发，为临床管理提供个体化方向。
